MEMBERSHIP GUIDELINES
- The patient portion of the bill is due the day of service.
- Cannot be used in conjunction with another dental plan or financing program.
- No refunds of premiums will be issued at any time if the patient does not use their membership plan.
- Cannot be used for referrals to specialists or any other general dentist office.
- Our Guarantee: no yearly maximums, no deductibles, no claim forms, no pre-authorization requirements, no pre-existing conditions limitations, no denied coverage, and no waiting periods (immediate eligibility).
